What is ketamine?

Ketamine is classified as a general anesthetic. In that role, it has been used in surgery for decades and has a strong reputation of safety. Ketamine is so safe, it’s often used during children’s surgery and for adults who can’t tolerate other anesthetics.

Ketamine is a dissociative anesthetic, which means that at the right dose, it induces a pleasant dream-like state that helps you relax and allows your brain to retrieve memories and events. During KAP, you can access deep emotional issues and discuss them with Dr. Khinda.

Ketamine is a successful depression treatment, anxiety treatment, and can help with PTSD, OCD, and chronic pain.

How does ketamine treat depression?

Depression is often treated with antidepressant medications. When these drugs work, they’re life-saving. However, here’s the challenge: antidepressants fail to improve depression in up to 35% of all patients. For those struggling with treatment-resistant depression, ketamine-assisted psychotherapy offers renewed hope. Dr. Khinda safely and effectively uses ketamine-assisted psychotherapy to help patients who haven’t responded to traditional treatments.

Ketamine is highly effective for severe depression and suicidality, particularly noted for its rapid action in patients with treatment-resistant depression.

This versatile medication has varied effects based on its dosage. High doses are used for anesthesia, while lower doses are potent pain relievers and mood stabilizers.

One of ketamine’s unique benefits lies in its impact on glutamate, a vital brain chemical that regulates mood. By restoring normal glutamate levels, ketamine helps improve mood and potentially repairs damaged nerve connections in the brain.

Treatment-resistant depression occurs when symptoms persist despite trying at least two different antidepressants. Ketamine infusions, administered intravenously, provide relief for many within days, offering an alternative for those who had little success with standard therapies.

Its effectiveness led to the FDA’s approval of SpravatoⓇ in March 2019. Spravato, a nasal spray containing esketamine (a ketamine derivative), is administered in certified medical offices to enhance accessibility for patients in need.

 

What is ketamine-assisted psychotherapy?

True to its name, ketamine-assisted psychotherapy (KAP) combines ketamine with therapy. When you come in for the treatment, you will begin by having a talk with Dr. Khinda and engage in relaxation exercises. After that, you’ll have a choice to listen to music or just relax. From there, you will take the ketamine either orally or with an injection. During the next 45-60 minutes, your brain will experience ketamine, and it can take a variety of forms. The doctor will be there to support you, but many patients recall an out of body experience.

 

During the Ketamine experience, you can experience deep emotional issues. With the doctor’s guidance, you will experience connections with your emotions and thoughts. You will see how your thoughts trigger emotions. With your new insights, you can drive results by changing negative patterns that lead to negative thoughts.

 

During the third hour, you will gently come out of the Ketamine experience, and you spend an integration hour with the doctor to apply what you learned to your life. The doctor helps you to understand and process your revolutions.

It is recommended to undergo at least three ketamine sessions.

 

Combining therapy with ketamine is the key to long-lasting results because you learn how to control your emotions and behaviors. Unlike relying on IV ketamine, you can prevent depression without ongoing doses of the drug.

The practice is considered ‘out-of-network’ for all insurance panels. Dr. Khinda will provide you with a ‘superbill’ which you can submit to your insurance company for possible reimbursement. You will be responsible for all communications with your insurance company.

By not relying on insurance for payment, patients are provided with a superior level of care in terms of the duration and frequency of appointments, flexibility of scheduling, and independence and flexibility in shaping the treatment plan, as well as maximum privacy and confidentiality with your personal health information.

Dr. Khinda’s fees are comparable to other child, adolescent, and adult psychiatrists in NY, NJ, and CT. Please contact Dr. Khinda directly to discuss the rates.

Payment is collected at the time of the session.

All credit, debit, HSA, and FSA cards are accepted. Personal checks or cash are not accepted. You will be provided with a receipt and an invoice for each session. There are NO ADDITIONAL FEES for brief telephone calls, prescription refills or sessions involving family or significant others. However, controlled substances will not be refilled without coming in for an appointment. This is done both to ensure patient safety and to meet DEA requirements.

If you do not show up for your scheduled appointment, and you have not notified of the cancellation at least 24 hours in advance, you will be required to pay the full cost of the session.

Dr. Khinda offers a complimentary 15-minute consultation via phone.

Once a mutual agreement to engage in treatment takes place, Dr. Khinda will schedule a 1-hour initial session for a deep exploration of your concerns. Initial appointments with children and adolescents will be scheduled for 90 minutes for collaboration of care with parents, schools, pediatricians, etc.

The initial appointment is 1 hour for adults and 90 minutes for children & adolescents. All subsequent therapy and psychopharmacology sessions are up to 45 minutes or less. Sessions may be shorter upon request, however, all sessions are the same fee.

Dr. Khinda will collect a thorough medical and psychiatric history and address the issues that brought you in.  At the end of the session, Dr. Khinda will offer recommendations and produce a treatment plan.  Sometimes this may be a referral to another treatment setting. Medication prescriptions are sometimes but not always written in the first session.

In preparation for the initial evaluation, please bring the following paperwork (if applicable):

  • Neuropsychological/Psychological Evaluations
  • Academic Evaluations (IEPs) and Report Cards
  • Prior psychiatric evaluations
  • Recent physical exams and specialty medical assessments
  • Recent bloodwork/laboratory results
  • List of current prescriptions and over-the-counter medications
  • Contact information for PCP, current mental health care providers, or school personnel

For medication only, sessions occur every 1-3 months but are more frequent at the beginning of treatment, for instance, they can occur every two weeks for the first 1-2 months of treatment.  For psychotherapy sessions, the frequency can be weekly, twice weekly, or every two weeks.

If you are running low on your medications, please contact Dr. Khinda a few days before you are due for a refill. Pharmacies may not have your medication in stock, which can cause a delay depending on the pharmacy or medication.

If the decision is made to start a new medication, this may require prior authorization. Please allow 7 business days for prior authorizations.

In the event of a life-threatening emergency, please call 911 or go to the nearest emergency room.
